Concerns about Your Care
If you have concerns about your care at the Southern Arizona Center Against Sexual Assault, there are many ways to be heard: 

  1. Talk openly with your treatment team. You can also ask to speak with the SACASA Director.
  2. Complete the SACASA Perceptions of Care and Satisfaction Survey online here
  3. Provide feedback in our online "comment card" by clicking here
  4. Call CODAC's safe and confidential Solution zone to speak with a CODAC team member who will work quickly to get you the answers you need. Call (520) 343-2225.
  5. For fraud and abuse concerns, send a confidential fax to CODAC's Corporate Compliance Officer at (520) 202-1845. 
  6. Review the grievance and appeal process posted at SACASA. 
  7. To file a discrimination complaint
